About the Project:

Graphics Unlimited was approached to help the Heart Foundation put together their Toolkit for reducing heart failure readmissions. Designed specifically with Victorian health services in mind to understand, act and provide targeted solutions to address heart failure readmission rates based on the patient’s needs, health service pressures and best-practice recommendations.

This involved the design and layout of the publication, as well as the creation of infographics based on supplied statistics.

On completion of the design stage Graphics Unlimited also produced the printed publication that involved the application of Gloss Spot UV on the cover and folded pull-out inside page sections.
